For Newly Hired and Existing Employees without PIN yet (EMPLOYED SECTOR)
- Fill out PhilHealth Member Registration Form
- Together with your pre-employment requirement; the Philhealth Member Registration Form should be accomplished and has to be submitted to the recruitment section of your new employer.
- This form maybe given as part of the pre-employment or can be requested to your nearest Philhealth Branch. However the form should be submitted to the HR and not directly to PhilHealth since it will require certification from the employer. Attach clear copy of supporting documents for qualified dependents declared.
- This form should be submitted along with the necessary documents.
- Photocopy of Birth Certificate
- Photocopy of Marriage Contract (if married)
- Photocopy of Birth Certificate of your dependents (offspring not over 21 yrs. old)
- Copy of Senior Citizen of your parents (if 60 y.o already)
- dependents including parents can be declared later to your employer as amendment;
- you may re-file or re-submit PMRF to declare your dependents so as not to delay your registration if you don’t have yet supporting documents for them.
- you will only need your Birth Certificate/ any valid IDs in order to secure PIN/ Philealth Identification Number
- Complete details of documentary requirements may be seen at the back portion of the registration form.
- Submit to the HR Department for them to forward the above documents to PhilHealth together with ER2 Form (Report of Employee-Members).
- Your PMRF as part of the pre-employment requirements should only be processed by your employer.
- It is your employer’s responsibility to have you registered and reported under their company name and PEN (Philhealth Employer Number)
- PhilHealth will send the PhilHealth Identification Card or PIC and a copy of the Member Data Record to the member, through the employer.
- Your registration form in return will be given copy of your Member Data Record/ MDR as proof of your registration; this should be given back to you for your reference or record.
- The MDR is also part of hospitalization requirement in case you will be utilizing your PhilHealth Benefit / Insurance.
- Report your PIN to your employer for them to indicate the same in their ER2.
- It is still necessary for you to accomplish PMRF in order to update your record, specially your employer data and membership category.
- Same Form (PMRF) will be utilized;
- For Sponsored (Indigent) Member – Your Local Government unit will do the registration for you. They will also shoulder your premium/ your PhilHealth Monthly Contribution; Philhealth Card will be provided as a proof for being categorized as sponsored member.
- For Lifetime Member (Retiree/ Pensioner) – No registration will be done; those who will fall on this category are those employed or individually paying member who already reached 60 or retiring aged and has contributed 120 months of premium.
- For Individually Paying Member – One can only proceed with his/ her membership/ registration after paying his/ her quarterly premium contribution of 600 Php (current rate as of Y2014).
